Summary: EXECUTIVE SUMMARY: This study awas conducted to determine the least mastered skills of Grade 5 pupils in Science as a basis for instructional material development. In determining the least mastered skills of the pupils in Science, the overall result of first, second and third periodical examinations were used. The findings showed that the least mastered skills of Grade 5 pupils in Science with respect to their first periodical examination are recognizing the importance of recycling waste materials and recognizing the importance of reducing waste product. With regard to their second periodical examination these are naming plants gorw from tubers and naming plants that grow from roots, and pertaining to third periodical examination these are identifying the materials that can block, absorb or transmit sounds and why some materials are good conductor of electricity. The developed instructional material was in a form of module. It consists of lesson, title, objectives, discussion of topic, generalization and different activities. Some parts of every lesson were creatively described using the terminologies such as, "Let's study this", which pertains to the discussion of topic; "keep in Mind" for the generalization; "Do This" for the activities. The developed instructional material was validated by the experts in Science from Grade 4 to Grade 6 and they gave some suggestions for the improvement of the instructional material. The suggestions were integrated on it which led to its validity. The general rating for the evaluation was outstanding. Based on the summary of findings and conclusion drawn, the following recommendations were recommended; utilize diagnostic test as a basis for instructional material development; and conduct a follow up study using the developed instructional material and determine its effectiveness.
