Dissertation note: Thesis (Bachelor of Technology major in Electronics Technology) -- University of Rizal System-Morong. Summary: EXECUTIVE SUMMARY: This study aimed to design and develop a Computer-Based School Forms Vending Machine. It was conducted at the University of Rizal System-Morong Campus during the school year 2016-2017. Its focused is on the development of Computer-Based School Forms Vending Machine which can be used by the College of Industrial Technology students. It automatically dispenses student clearance with printed mother organization and sub-organization logos and other commonly needed school forms such as course prospectus, completion form and adding/dropping/changing of and also the request for another copy of student clearance form in case of loss. The estimated cost of materials used in production, estimated cost labor and numbers of working days consumed during the construction of the device were carefully analyzed and discussed in this study. The operating principles, construction procedures, tools and equipments and function of each part are included in this study. The researchers used the developmental and descriptive methods of research and chose five (5) Electronics and five (5) Information technology (IT) experts as they were capable to determine the level of performance in terms of input selection, response time and output accuracy, and the level of acceptability in terms of accuracy, applicability, user-friendliness, workability, completeness, portability and modifiability. The study was composed of input, process and output that were utilized in the development of the project. The feedback was also considered. Based on the intensified treatment of the data gathered, the Computer-Based School Forms Vending Machine was designed and constructed out of locally available materials. The level of performance of the developed device in terms of input selection and output accuracy had a 100% success rate and varied in terms of response time. As to the level of acceptability of the developed device, all are strongly accepted. In the light of the summary of findings, it is concluded that the developed Computer-Based School Forms Vending Machine can operate according to what is supposed to perform and satisfies the user's need in accomplishing school forms specifically student clearance with mother organization and sub organization, request for another copy of student clearance, adding /dropping/changing of subject form, completion form and course prospectus form which will minimize time consumption and faster by the automation process instead of manual process. The developed system can be transferred from one computer system to another. Also, it was acceptable to the experts on the acceptability of the system with regards to different variables. Based from the results of evaluation and conclusion on the developed Computer-based School Forms Vending Machine, the following were hereby recommended: the developed school forms vending machine can be implemented in order to have an automated system of accomplishing school forms within the University, the developed system can be a crystal report for more user-friendly command buttons, and also the developed system can be centralized, accessible for all the students, and connected to the registrar and cashier.
