Summary, etc. EXECUTIVE SUMMARY: This study aimed to determine the extent of the health status and school performance. It was conducted in selected elementary school in Morong, Rizal namely Calero-Lanang Elementary School, Timoteo A. Reyes Sr. Elementary School, Roman Tantiongco Memorial Elementary School, Maybancal Elementary School, Tomas Claudio Memorial Elementary School during the academic year 2015-2016. The researchers used descriptive type of research specifically survey method to gather the needed information. The instrument was subjected to content validation by the experts in the field of Guidance and Counseling and related areas. The profile of the respondents were used as the variables were considered in the study like health status as to their eating habits and physical condition and aspects of attendance, recitation/participation, activity involvement and result of examination as to school performance were considered too. The data were gathered, tabulated and interpreted. Frequency and percentage were the statistical tools used to determine the profile of the respondents in terms of age, sex, monthly family income, number of siblings and parental status. Weighted mean was used to determine the extent of health status and school performance of grade IV pupils. To determine the significant differences on the extent of health status and school performance of the respondents in terms of their profile, one-way analysis of variance (ANOVA) was used. Findings of the study revealed that 94.0 percent of the respondents belongs to 9-10 years of age while 52.0 percent are female, 60.0 percent has a monthly family income ranging to 5,000-10,000. 44.0 percent with to 3-4 numbers of siblings. 81.0 percent are married. Findings of the study revealed that with respect to health status in terms of eating habits has obtained mean of 2.60 with a verbal interpretation of "sometimes", while with respect to physical condition has obtained mean of 2.49 with a verbal interpretation of "seldom". The findings of the study revealed that with respect to school performance in terms of attendance it obtained mean of 2.96 with a verbal interpretation of "sometimes", recitation/participation, it obtained mean of 3.50 with a verbal interpretation of "often", activity involvement, it obtained mean of 2.57 with a verbal interpretation of "seldom: and result of examination, it obtained mean of 3.16 with a verbal interpretation of "sometimes". Based on the findings integration to class discussion on the importance of healthy foods should be done. Other school related activity may be done that would promote students intellectual and physical skills. Provision and enhancement of feeding program and physical activities may be conducted and another study may be conducted using other variables.
